SUMMARY:

A member of the ACE NURSES nursing staff who is highly involved in the delivery of quality nursing care. The Employee will work in conjunction with the Supervisory Nurse(s)/ Administrator and Administrator’s Staff to meet the client's needs and/or resolve problems.

RESPONSIBILITIES:

DUTIES:

1.  Provides nursing care according to established policies and procedures.

2.  Arrives to work on a timely manner and keeps absences down to a minimum.

3.  Adheres to the dress code and comes to work prepared (watch, pen, badge, nursing smock)

4.  Reports on and off duty to the Supervisory Nursing and Administrator.

5.  Receives report and client’s assignment from the Supervisory Nurse and satisfactorily completes the assignments by the           end of his / her shift including documentation of care.

6.   Reports to the Supervisory Nursing and Administrator of any unusual findings noted while giving care.

7.   Completes additional task as requested.

8.   Assumes responsibility for all clients entrusted with, providing watchful supervision and oversight during absence of the             caregiver(s) Alerts the Supervisory Nursing / Administrator immediately of any procedures or duty that needs                clarification or instruction.

9.    Adjusts to charges in the routines to meet client/ resident's needs.

10.  Adjust and adheres to changes in policies and procedures as revised by the Agency.

11.  Checks the master schedule to see when he/she is scheduled to work.

12.  Requests time off, and vacation according to the Agency policies and procedures.

13.  Clock in/out, document notes sign and have timesheets electronically

14.  Works holidays according to Agencies’ policies and procedures.

2. CLINICAL DUTIES:

1. Practices proper infection control techniques. Follows Agency infection control policies and procedures.

2. Maintains meticulous personal hygiene and grooming of the client. Give showers, bathes, and shampoos clients as assigned. Besides bathing and incontinent care daily AM and PM care includes oral hygiene, as well as denture care, shaves for men daily and women as needed, inspecting fingernails to ensure they are clean and well-trimmed, washing inspecting skin for red or open areas, bruises, skin tears, reports to the Supervisory Nurses / Administrator.

3. Toileting and/or incontinent care is provided according to the Agencies’ policies and procedures.

4. Encourage client to participate in active ROM when possible and perform passive ROM according to the agency policies and procedures (if asked to do so in the care plan).

5. Clients are assisted before, during, and after meals by placing clients in an upright position, ensuring teeth are in, glasses are on,  preparing meals and washing dishes .Give client as much assistance as required determine in the care plan (cut meat, open containers) encourage the client to eat and/or feeds resident/client as indicated on the assignment sheet. Record client/ resident's intake and repost to nurse if client/resident's intake is low meal is refused, etc.

6. Assist with Transfers client according to method indicated on assignment sheet. Follows policies and procedure on body mechanics and use of resident lifts. Notify nurse if in doubt of client/resident's capabilities or questions about transfer method to be used.

7. Assist clients in and out of bed, chair, and/or wheelchair, assisting with walking, encouraging physical activity, assisting with simple exercise program established by nurse or therapist, if appropriate

8.Obtains the client's vital signs and weight according to shift routine or as directed by Supervisory Nurse as indicated in the care plan.

9. Observe and report changes in member’s condition, meal consumption, food storage and cooking equipment failure to the supervisor.

10. Maintain current progress notes indicating changes in the member’s condition, problems that hinder service delivery, and additional needs of the member complete the Member Service Record after each visit for each member and forward it to the PSS supervisor as required

11. Answers call promptly for any client and addresses the resident's needs

12. Responds immediately to alarms, Code Alert or door alarms.

13. Assisting with self-administration of medication, reminding member to take medication

14. Ensures safety of all clients assigned.

15. Immediately reports to Supervisory Nurse/ Administrator any unusual occurrences or injury of a client.

16. Immediately reports hazard/malfunctioning equipment to the Supervisory Nurse/ Administrator.

17. Picking up prescription medications, arranging trips to the doctor, assisting with scheduling medical appointments

18. Monitor members and their environments to address and report issues that impact members’ health, safety, or        welfare.  Member observations include:

             • safety in the home

             • changes in the member’s physical or emotional condition

             • changes in the member’s support systems

19 .Accepts changes in assignment as designated. Works in areas according to daily assignment.

 

5. NON-CLINICAL DUTIES:

1. Care for the client’s immediate environment vacuuming, sweeping, dusting, mopping emptying trash and garbage for the member doing laundry for the member, changing linens for the member, cleaning rooms for the member, cleaning range for the member and ironing for the member

2. Assist with scheduling Medical appointments.

4. Accompanies clients to appointments at MD offices as directed.

5. Accompany clients to grocery shopping or other activities.

6. Encourage member to make decisions and to remain as independent as possible

7. Accepts changes in assignment as designated.

6. COMMUNICATION:

1. Communicates in a pleasant, polite, and helpful manner to residents, visitors, coworkers.

2. Works as a team member with co-workers.

3. Welcomes new employees and assists in their orientation by acting as preceptor.

4. Directs resident or visitors concerns to the Supervisory Nurse/ Administrator or appropriate department head.

5. Exhibits a professional manner at all times.

6. Follows Employees Code of Conduct Policy.

7. DOCUMENTATION:

1.Records vital signs, weights, intake, and output, percentage of meals, bowel movements, bladder assessment, and ambulation list on the proper forms or in the EMR. Notifies the Supervisory Nurse of abnormal findings.

2.Documents care in the Electronic Medical Record (EMR)

 

8. INSERVICE TRAINING

1.Attends State and Federal mandated in-service annually.

2.Attends in-service demand mandatory by the Agency.

3.Attends at least 8 hours of in-service training in a calendar year as required by regulations.

 

Other information:

 

EDUCATION:

 

High School education or GED

Certified according to State of Georgia Public Health Code regulations.

 

QUALIFICATIONS:

 

1. Ability to read and write, follow verbal and written instructions, and complete legible written reports of care given

 

2. Completion of one of the following training requirements:

 

Documentation of nurse aide certification from the Georgia Department of Community Health, Healthcare Facility Regulations Division; or

 

3. Successful completion of a competency examination for nurse aides or of a health care or personal care credentialing program approved by the Healthcare Facility Regulations Division, Private Home Care Providers Section, or

4. A total of 40 hours of training.  The aide receives at least 20 hours of this training prior to caring for members.  The aide completes an additional 20 hours of training within the first six months of employment.

 

SPECIAL_SKILLS:

CPR/FIRST AIDE

Must have valid TB

Driver’s license or other form of identification

Demonstrates excellent communication and personal skills.
